Commonwealth Home Support Program

Target Group:  65 years or oler, or 50 years or older and identify as an Aboriginal and/or Torres Strait Islander person.

Service description:

The Commonwealth Home Support Program is for older people who are mostly - but not completely - able to live and cope on their own. The program also assists carers. Commonwealth Home Support Program services provided at home may include: domestic assistance; personal care; home maintenance; home modification and nursing care.

Target Group: 

People with an impairment that is likely to be permanent and the impairment makes it difficult to take part in everyday activities. This includes people with permanent and significant disability, and people with severe and persistent mental illness in limited circumstances. People need to be aged less than 65 years when making an access request.

Service description: The NDIS will deliver a life-long approach to support people with disability through individualised planning processes to identify the reasonable and necessary supports participants need to enable them to achieve their goals.
